Aboriginal Homeless & Fringe Dweller Support Service (AHFDSS) Worker

The purpose of this position is to provide immediate and rapid response to Aboriginal & Torres Strait Islander chronic rough sleepers.  This position will form part of the Social Support Unit and overall Bega program area’s that provide assertive outreach, triage and support with the aim of successfully transitioning people to safe accommodation and housing linked together with long term Emotional Social Well Being and Clinical Health programs as needed.

QUALIFICATIONS, SKILLS, EXPERIENCE AND KNOWLEDGE

ESSENTIAL:

  • A qualification in Community Services or a proven commitment to issues surrounding homelessness with experience working with vulnerable people;
  • A thorough knowledge of and experience in homelessness;
  • Well-developed oral and written communication skills, including the ability to relate well to staff and to liaise effectively with key stakeholders;
  • Demonstrated knowledge and understanding of cultural factors and social determinants particularly relating to Aboriginal & Torres Strait Islander Health;
  • Demonstrated commitment to confidentiality and a high level of accuracy;
  • Sound skills in problem-solving and the ability to effectively determine priorities to ensure deadlines are met;
  • Work effectively as a member of a team in an environment of competing priorities;
  • Current Police Certificate (within 3 months) and Working with Children Check;
  • Current WA “C” Class Drivers Licence; (manual required)
  • Ability to work outside of normal business hours.

Position Term: Ongoing, 37.5 Hours per week

Summary of the Position:

The ACC has the primary responsibility of connecting with local Elders, older Aboriginal and Torres Strait Islander people and their families to raise awareness of their aged care entitlements. 
The overarching function of the ACC is to ensure that support for clients is organised, sequenced and supported to enable effective access to appropriate aged care services both internal and external to the ACCHO.
The ACC will work closely with the Aged Care Support Coordinator. They will also help determine the level of local demand for aged care services, identify existing aged care services, any service gaps in the area and continue advocacy work with clients as required.

Community Engagement Worker

To provide and facilitate support for our clients to access relevant services to address homelessness, substance misuse, violence and social dysfunction. To promote individual and community wellbeing, reduce substance abuse through the provision of referral and access to culturally appropriate alcohol and other drug prevention, education, treatment, rehabilitation and aftercare services for the ATSI people of the Goldfields region, including Ninga Mia and Boulder camp. Tobacco Action Worker

This position will assist in the delivery of a culturally appropriate smoking cessation program, tailored to the needs of the communities within the Goldfields region. This position forms part of a multidisciplinary team and will contribute to promoting, undertaking and supporting a range of programs, activities and workshops regarding strategies to address quitting smoking; and contribute to the development and provision of community education and engagement regarding the effects of tobacco and supporting healthy living.
